Dental Referrals

This website is provided for non dental professionals, carers and others with legitimate clinical relationships to patients to refer to selected dental services in Greater Manchester, Cheshire and Merseyside.

You should read the referral guidance here before considering a referral, and in order to access the form you must have your password. Your local authority lead for LAC will be able to provide this to you.

We enable non dental professionals and carers the ability to refer to Special Care Dental Services. Prior to making a referral you should contact your local Community Dental Services and obtain a passcode.

We enable non dental professionals the ability to refer to Urgent Dental Care centres across Greater Manchester and Cheshire & Merseyside for individuals with evacuee status.
